In this, the final episode of Season 2 of The Whole Story, we explore what culture is, what sets and defines it and its impact on the individual, team, organisation and patient safety. How is “the way we do things around here” ethos created and embedded, who or what can affect it and what does it mean for our workforce. Could paying attention to, and shaping culture revolutionise the way we work and what role does a “systems thinking” philosophy play in this. We offer practical tools to use to review the culture in our teams.
Paul and Sarah discuss the following aspects of organisational culture:
- Defining organisational culture and subculture and exploring the factors which influence them, including leadership
- The evidence base around organisational culture
- How culture is assessed, understood and changed
- What can the individual/leader/organisation do about a “bad” culture?
- What happens when different cultures interact?
- Is there a link between QI and culture?
- What is systems-thinking mindset/philosophy?
- Practical tools for reviewing and changing your workplace culture
Dr Sarah Luty has been a GP for 20 years, working most of that as GP and trainer in Kilmacolm. She worked with the patient safety team at NES for over 10 years and was the QI champion for the RCGP.
Sarah recently worked as associate postgraduate dean for primary care simulation at NHS Education for Scotland and is currently the assistant director of CPD connect developing educational resources and events for the primary care team.

Professor Paul Bowie is a chartered ergonomist & human factors expert, patient safety scientist, and medical educator with over 30 years of experience leading and collaborating in research, innovation and educational development to improve the quality and safety of healthcare in the UK and globally. Paul is Programme Director (Safety and Improvement) with NHS Education for Scotland based in Glasgow, Scotland and he is also Professor of Healthcare Human Factors at the University of Staffordshire in England.

In this episode we hear from Professor Nitin Gambhir about his journey of living and working in different cultures as well as:
- The impact of culture on interacting with patients and colleagues
- The impact of culture on consulting for trainees, educators and patients
- What practice teams can do to champion cultural difference

Nitin is the Postgraduate Dean, Lead Dean Director for General Practice, Public Health, Psychiatry and BBT, NHS Education for Scotland and a GP in Glasgow. Nitin has experience of living and working in different settings, of training and supporting international medical graduates through GP training and in devising and implementing policy relating to training across Scotland. Nitin is an examiner and EDI adviser for RCGP. He is passionate about equality, diversity and inclusivity and supporting IMG colleagues.

Elaine is a GP in Lothian. She is an Associate Adviser with NES, as Module Development lead for PBSGL and lead for the Whole Story Podcast series.
Elaine is passionate about professional development and has held roles in undergraduate and postgraduate medical education at both local and national levels with the RCGP and NES. She has a decade of experience as lead for Clinical Communication at Glasgow Medical School.
Elaine is an International Coaching Federation globally accredited Coach. She has a Masters in Clinical Education, whose research focus was professional identity development.

In primary care we deal with uncertainty every day, but does it mean the same thing and feel the same way to everyone? What does uncertainty do to us and the way we interact with patients and deliver care? Are there strategies we can deploy to manage uncertainty and our response to it. How do patients manage uncertainty and share decision making in the face of it? What can other industries teach us about uncertainty and how to approach it?
In this episode Elaine and Lindsey discuss the following:
1.What is uncertainty?
- What is the impact of uncertainty on clinicians and patients?
- What if we are uncomfortable with uncertainty?
- How can we help patients deal with uncertainty?
- Does uncertainty exist in other sectors/industries and how do they deal with it?

Professor Lindsey Pope is a GP, Professor of Medical Education and Dean of Internationalisation (MVLS) at the University of Glasgow.
She is the Director of Community Based Medical Education at the Undergraduate Medical School.
Lindsey is Associate Postgraduate Dean for Academic Medicine at NHS Education for Scotland and Chairs the Specialty Training Board for General Practice, Public Health and Broad Based Training.
She is Director of the Scottish School of Primary Care and the Specialty Adviser for General Practice to the CMO for Scotland. She is an elected member of the Medical Schools Council ED&I Alliance Board .
She is a GMC Education Associate with involvement in regional and school reviews, new school approvals and most recently as a Team Leader for the CPSA component of the new national MLA (Medical Licensing Assessment).
She is an Examiner for the Royal College of GPs as well as an approved GP trainer and has been involved in teaching and training across the continuum of medical education.
She has a Doctorate in Health Professions Education and is a member of the Scottish Medical Education Research Consortium (SMERC) Project Board. She is a member of the Scottish Doctors' Wellbeing Study group and sits on the Editorial Board of Virtual Primary Care. She supervises Masters and Doctoral students in Medical Education.

Part 2: Triggered by her own medically unexplained symptoms and a feeling of helplessness when working with patients experiencing the same Kath sought out a new way of consulting. From this emerged a route back to hope for patients, greater fulfilment and energy from work and a model which is gathering momentum both in her practice and in her business. Kath Jones talks about the concept of Sustainable Healing, as a facet of realistic medicine.

Dr Kath Jones, NHS GP, Lifestyle Medicine Physician & Founder of Wild-Ness

Kath has a background in general practice and academia. She moved to Highland in 2016 to take up a post as an Associate Medical Director. After returning to clinical work in 2019 she struggled with brain fog and a collapse in confidence, prompting her to consider leaving medicine. Rediscovering a love of exercise and learning after a short period of coaching inspired her to re-train as a coach, lifestyle medicine physician and found Wild-Ness Retreats. She combines this venture with work as a GP at the Alness & Invergordon Medical Practice where she integrates western acupuncture and lifestyle medicine into her NHS consultations. Kath is launching a new movement next year called The Sustainable Healer to promote a shift in practice towards sustainable healing which helps patients, practitioners, and the planet.
